Holy Trinity provides a broad range of skilled nursing and rehabilitation services while maintaining the highest standard of personal care for residents.  This care is delivered by staff who provide an environment that is warm, compassionate and supportive.
The center presently includes 113 beds of skilled and rehabilitative care and a new subacute unit,  with 17 beds, whose specially trained staff of RN's, LPN's, Case Manager (RN), Nurse Manager (RN) and CNA's provide transitional care.

Rehabilitation services are provided 7 days a week by Physical Therapists, Physical Therapy Assistants, Speech/Language Pathologists, Occupational Therapists and Certified Occupational Therapy Assistants.

Holy Trinity provides Skilled Nursing and Rehabilitation for the following diagnosis:

Post Surgical Care Orthopedic Care including:   - Hip Fracture/Replacement   - Knee Replacement Surgery   - Spinal/Back Surgery   - Skeletal Traction Post Falls Gastric Disorders Infection Control IV Hydration/Antibiotics

Pneumonia Chronic Obstructive Pulmonary Disease (COPD) Congestive Heart Failure (CHF) Myocardial Infarct Coronary Artery Disease (CAD) Cornonary Artery Bi-Pass Graft (CABG) End of Life Care Pain Management Special Nutritional needs such as G-Tubes
